Health Requirements and Medical Forms

 

Please see all the Indiana health requirements for your student in the charts below. You may use the Immunization form linked below or turn in the immunization form your doctor's office prints out. 

All needed forms are linked below and should be submitted to school prior to the first day of school.

Vision: K

Physical Exam Forms: K, 6th, and 9th 

New Students: All forms that are required through the grade the new student is entering.
